What to do if you contract H1N1

If you experience symptoms of the H1N1, stay home from work and get immediate medical attention.

If you are found to have contracted H1N1 while on the job, file a CA-2. If you are refused a claim form, go to the DOL website and get the form. Fill it out and submit it to DOL with a note stating that your agency refused to provide one.
